Any contractor or subcontractor and/or its successor
shall be ineligible to submit a bid on or be awarded any public
work contract or subcontract with any state, municipal corporation
or public body for a period of five years when:
1.) Two willful determinations have been rendered against that
contractor or subcontractor and/or its successor within any
consecutive six-year period.
2.) There is any willful determination that involves the falsification
of payroll records or the kickback of wages or supplements.
